"Free of charge" still requires someone to pay. All of these are necessary for everyone but it becomes a question of quality and quantity.
"Free of charge" is actually a way to say the cost should be shared amongst everyone, through taxes.
With a progressive tax proportional to the income it would mean everyone contributes in proportion to their means, and everyone can live a healthy life.
